Please read carefully and sign at the bottom of the page indicating your authorization for haul out and or repair work to be done.  I authorize Five Star Marine to begin the repair process by authorizing work and any necessary materials to prepare an estimate. All estimates are subject to change after further inspection of the vessel. When hauling out or launching a boat, Five Star is not responsible for any damage on the bottom of the boat due to rot and soft fiberglass or anything that comes into contact with trailer bunks (any through hull fittings). Five Star is not responsible for anything out of its control, such as rain, sun damage, heavy winds, and any type of mother nature that might cause damage to the vessel. Five Star is not liable for any items left on the boat, theft, vandalism, boat rot, and soft fiberglass. In the event of default payment, Five Star is entitled to a mechanic’s lien to secure the total repair charges under section 70.001 of the Property code. “THIS VEHICLE IS SUBJECT TO REPOSSESSION IF YOU DO NOT MAKE FULL PAYMENT” upon completion of work.
